The above documents must be bound in one volume with application and business registration.
Upon receipt, the competent authority will evaluate.
For the results of the evaluation that are not eligible will be extended for a maximum of 60 days to complete the equipments and facility.
Time passed but those who can not meet the standards will be monitored and not be active until certification.
The form used in the above procedure is also issued conjunction with Circular 26/2012/TT-BYT.
